NorthPoint Health & Wellness Center (NorthPoint) is a comprehensive health and human services agency located in the heart of North Minneapolis.  NorthPoint is comprised of two organizations: a community services nonprofit with a broad array of social and human services, and a full service health center that includes medical, dental and behavioral health clinics.  The two entities share an integrated mission and report to a common CEO and Community Board of Directors.

The mission of NorthPoint is “actively partnering to create a healthier community.” NorthPoint seeks to reduce health disparities, improve health outcomes, and enhance the overall quality of life for all residents of North Minneapolis by providing high quality medical, dental and behavioral health care, along with integrated social services such as its food shelf, housing resources and domestic violence services — with respect, dignity and sensitivity to a culturally rich and ethnically diverse community.

One of NorthPoint’s ongoing key strategic goals has been to increase the community’s access to available health and human care service as a way of addressing many of the health and socio-economic disparities in the community.  In response to community, partner and patient/client input, it hopes to address community needs that have been identified by evaluating its current practices to be sure it is providing efficient and effective services and continually seeking out additional partnerships, funding and other resources that will allow us to address the community identified needs.

Hosting a VISTA would allow NorthPoint to leverage its health disparities work through various initiatives and projects with a primary focus on builiding capcity and sustainability around the Eliminiating Health Disparities Intitiative (EHDI) and Outreach Deparment activities such as assisting in the research, development and implementation of culturally appropriate programming aimed at addressing health disparities, strengthening relationships with community and academic partners, and participaing in upstream intervenitons through the NorthSide Healthy Eating Project.
